Level 2 - A systems approach to learning from patient safety incidents

Full course description

This course is aimed at those who lead investigations and other learning responses and those in Patient Safety Incident Response Framework (PSIRF) oversight roles. The course meets the minimum training requirement of this specific element of the new PSIRF. The course includes:

  • An introduction to complex systems, systems thinking and human factors
  • Investigation practice such as interviewing, capturing work as done, use of a systems framework (SEIPS), data synthesis, and report writing
  • Developing effective safety actions and recommendations
  • Engaging and involving those affected by patient safety incidents

This is a self-directed online learning course with bitesize modules for you to learn at your own pace over 6 months. We will hold weekly Q&A sessions every Thursday 10 - 11am from 15th September 2022 until 15th December 2022. These will be held within Canvas via Big Blue Button. 


All of the modules will be released on Monday 12th September 2022 at 0900 and you are expected to work through the modules at your own pace. Please note in order to receive your certificate of attendance you must complete the quiz at the end of each module by Sunday 12th March 2023.
